For each claim: state whether it is Observed or Unknown, the confidence for observed claims (high/medium/low), and the source. Say what evidence basis the diagnosis run had (grounded read / direct scan only / unavailable). For each Unknown, name the single most useful verification.
What a good answer contains
Categorical Observed/Unknown states, confidence labels, cited sources, and a concrete verification step per Unknown.
What it must not claim
Grades, maturity scores, or that Unknown means zero or failure.
